33, Single, no children. I had my first appointment/exam with Dr. Hunkele and she asked if I had any questions. I asked about sterilization and she asked me what I knew about the process and what my feelings were. I answered her questions and she approved it right then and there. I signed a form stating I understood that it was permanent. I had it scheduled for a month later. She is also a surgeon and did my surgery herself! Out of pocket cost was $1,000.
